Clutch/tranny issues... maybe? *RESOLVED*

The other day I noticed that 1st gear was a bit more difficult to go in and reverse is grinding every once and a while. My clutch engagement has always been really low on the pedal and seems to have moved just far down enough where when the pedal is to the floor the clutch is either fully disengaged or partially disengaged - it's inconsistent. If I put the car in first and two step (4,500RPM) the car lurches forward a little (it seems to "surge" a bit but only moves forward an inch or so).

Fluid in the reservoir is full and I followed proper instructions to set the clutch pedal (found the point where the bleeder was blocked off and backed it off a little bit). I don't seem to be loosing any fluid from the slave but there's a hint of fluid on the outside of it (hard to tell if it's brake fluid or oil) but not a lot by any means. The fluid isn't super clean though.

Any thoughts on what might be going on? Only thing I haven't really tried is bleeding the clutch.

Car is a 2G GSX, Shep Stage 4, PTT twin disk, Isuzu slave cylinder, NEO RHD fluid previously, I just changed to Amsoil 75-90 but it feels the same.
