I already went through this once, exactly how you are describing.
The problems with having no reverse and no 5th sounds like you damaged the tranny, the causes for the damaged tranny are due to one or more of the items below. I damaged my tranny because of these things, hope it helps you.
Things to Check/replace:
VERY FIRST THING. Turn your adjustment screw on the peddle so to the right if your looking at the master cylender part of the screw, or the left if your facing the peddle. make sure it is almost all the way out. Do not take this step lightly, make sure you dont just skim over this because this can save you a shit load of a headache.Then try shifting it.
Go under the transmission where the slave cylender and make sure that the throwout bearing fork is either in the center, or towards the driver side. If its towards the passenger side then you either have a problem with the pressure plate, the clutch, or the shift fork, either way your taking out the tranny.
IF you are gonna take out the tranny check the clutch disk. Mine was destroyed. Check the pressure plate and the pivot ball. you might as well replace the throwout bearing just to be safe (and save a headache later on.
MAKE SURE the bolts that hold the tranny to the block are all very tight, especially the one that is just above the half shafts. (I had this problem, the bolt kept vibrating out and would cause the clutch not to shift due to the pressure plate pushing the tranny away from the motor. ) Make sure that this part of the tranny has no gap. it should be pressed up against the motor, This bolt is the only one that goes from the motor to the tranny, (as opposed to the bolts that go from the tranny to the motor)
Check these things and im sure you will find one of them problematic.

Clutch adjustment issues effect ALL gears, not just reverse.
Not nessesarily. When my bolts kept coming loose it affected reverse more then any other gear. DRASTICALLY. It would go into first and any other gear, (with little grind) but reverse was horrible.
He also said that the clutch engages right off the floor which is a tell tale sign of loose bolts or clutch adjustment.

What loose bolts? all are tight and i adjusted the clutch it didnt do anything for me.

I did for the reverse switch it was there, shortly after looking at that the reverse didnt work.

Take off the end cover, there are 2 36?mm nuts. Those are suspose to be tightened and then "dented" so they don't back off.

Well i did what you said i tightend the two bolts, they were loose alittle the middle one more than any, but there was no difference still no 5th just grinds and reverse is still locked out. Any ideas?

How is the shift movement? Do you have full range on the shifter assembly?

I got full range 1 thru 5th but reverse is locked out wich i really would like fixed i still drive the car every day with just 4 gears, 5th on the shifter locks in but theres nuthing there, i was thinking i could just pull the 5th gear, gear set off and put the one off of my bad transmission, on my bad tranny the intermediate gear shaft the bearings went out and it wore away some stuff and busted the case. Do you think i should try the 5th gear set swap?

Taking apart a transmition and putting it back together right is not an easy task. I would not recommend it unless you seriously know what you are doing, and have the right tools to do it.
