Internal Combustion is slowly but surely going to die out. I am thinking Direct Injection is going to be the last major "breakthrough" in engineering of the IC engine.

But there are many cool concepts that could theoretically work to help up the efficiency.

If you want a engine that fires on the up and down stroke, look no further then a steam engine.
