Hmm.. It sounds like it was an assembly error. Those nuts go in with the "big" side down on the gear. You usually want to switch them around. After install, did you "dent" the sides to the shaft?
The "gear" that fell out is actually your syncro (brass colored?), the retainer is there in the case to keep it down within the case.
We really don't get into transmission work much here. All of our dsm work goes out to shep racing. If the nut is stripped you need to order a new one from brookdale. Following my instructions above, you should beable to figure it out yourself.
Make sure you are using a good quality RTV when re-sealing the case.
