It is already spreading. It has already mutated. A few different strains have been discovered over the past 5-10 years and this most current strain has now spread from asia to eastern Europe, detected in humans. This new strain has not be discovered in the Americas.

Well, it is not exactly like that. The strain has been contracted in humans only from direct contact with infected birds. It has not been spread from human to human, that is the jump it would have to make to become an epidemic. So no, it has not mutated yet. Yes there are different strains and there have been for quite a while.

It has spend from one human to another human. But not any farther than that. Once it gets a little more mutated maybe it will be able to spread freely among humans. It could be very deadly with a mortality rate so far of about 50%. The plague has a mortality rate of about 15% and ebola that is 50-90% effective at killing humans , but requires direct contact with bodily fluids so it's a bit harder to acquire. The World will be in trouble when a high fatality rate virus that can spread easily through the air, like a flu, between humans hits.
